Licensed Practical Nurse Key Responsibilities & Duties • Supervise assigned staff and coordinate medical and psychosocial referrals for clients. • Act as primary medical case manager, conducting health checks and optimizing client outcomes. • Maintain medication administration records, controlled substance logs, and ensure adequate medication supply. • Participate in treatment team meetings and case conferences to share findings and coordinate care. • Provide emergency first aid and CPR assistance when required. • Educate clients on health topics and advocate for their health preferences and goals. • Collaborate with nursing leadership and team members to ensure comprehensive client care. • Perform related duties as assigned to support the assessment center's mission. Licensed Practical Nurse Job Requirements • Valid Licensed Practical Nurse (LPN) certification is required. • Minimum of 1 year of nursing experience; 2 years preferred. • Associate of Applied Science (AAS) degree in nursing or related field. • Experience in medical, psychiatric, and substance use care is highly beneficial. • Strong teamwork and communication skills to collaborate effectively with multidisciplinary teams. • Ability to manage medical records and medication administration logs accurately. • Commitment to providing compassionate care to medically vulnerable populations. • Availability to work onsite during scheduled hours with flexibility for emergencies.
