Care Coordination Director Key Responsibilities & Duties • Direct and evaluate departmental operations, including staffing, onboarding, and competency development to meet quality objectives. • Allocate resources effectively to achieve productivity and performance goals within the case management department. • Oversee care coordination and discharge planning processes, ensuring timeliness, quality, and proper documentation. • Collaborate with the Facility CFO and Division Case Management Leader to meet organizational expectations. • Assess and improve departmental performance through data analysis and sustainable improvement activities. • Establish working relationships with key stakeholders, including CMO, CNO, and ancillary service leaders. • Ensure compliance with policies, standard operating procedures, and ethical guidelines. • Lead initiatives to adapt to changes and coordinate efforts across multiple stakeholders effectively. • Perform additional duties as assigned to support organizational objectives. Care Coordination Director Job Requirements • Bachelor's degree in Nursing, Health Administration, or Business Administration required; Master's degree preferred. • Registered Nurse (RN) or Clinical Social Worker (LCSW) certification preferred. • Minimum of 3 years of acute care case management leadership experience required; 5 years preferred. • Proven ability to manage multiple priorities and work independently with minimal supervision. • Demonstrated expertise in case management, team building, and change management. • Strong communication and presentation skills with a record of delivering results. • Experience in performance improvement and operational process evaluation. • Ability to adapt quickly to change and coordinate efforts across diverse stakeholders. • Knowledge of compliance requirements and ethical guidelines in healthcare settings.
