Remote RN Case Manager (Costco Team) 📍 Remote 🕒 Full-Time | Monday–Friday 8:00am -5:00pm CST | No Weekends or Holidays | Occasional rotation to 10:00 AM – 7:00 PM CST Make a meaningful impact—right from home. Join our Costco team as a Remote RN Case Manager, where you’ll play a key role in helping members achieve better health outcomes through compassionate, coordinated care. What You’ll Do • Conduct telephonic (and occasional in-person) assessments to understand member needs • Develop and manage personalized care plans to improve health and wellness • Coordinate care across programs, providers, and resources • Identify risks and remove barriers to care using clinical judgment • Engage members through meaningful conversations to support their health goals • Collaborate with a multidisciplinary team to optimize outcomes Required Qualifications • Active, unrestricted multistate RN license in state of residence • 3+ years of clinical nursing experience • Strong computer skills (Microsoft Office + web-based systems) • Ability to work the required schedule Preferred Qualifications • Case management experience (integrated model) • Bilingual (Spanish/English) Education • Associates Degree in Nursing required • Bachelor's in Science & Nursing preferred Why Join Us? • Fully remote flexibility • No weekends or holidays • Opportunity to make a real difference in members’ lives • Supportive team and professional growth opportunities Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
