Job Title Registered Nurse – Case Manager Location Eureka, CA | Acute Care Schedule / Job Type Contract | Days, 5x8 Hours (8:00–16:30) | Every Other Weekend Required | Onsite Key Responsibilities • Provide case management services within an acute care hospital setting • Coordinate patient care plans, discharge planning, and utilization review • Document and chart patient information using Epic EMR • Float between like units based on facility need • Collaborate with interdisciplinary teams to ensure continuity and quality of care Required Qualifications • Active Registered Nurse (RN) license — Compact license must be in hand at time of submission • Minimum of 1 year of RN experience required; 2 years preferred • Acute care experience required • Proficiency with Epic EMR • Must commit to working 3 of the 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Must live at least 50 miles from the facility to be eligible for standard traveler consideration; locals within 50 miles are accepted under separate terms Preferred Experience / Skills • 2 or more years of case management experience in a hospital setting • Experience with utilization management and discharge planning • Familiarity with floating between sister or like units Why Join Us • Located in Eureka, CA — easy driving distance to 7 state and national parks, the rugged California coast, and the iconic Redwood Highway, offering an exceptional quality of life for outdoor enthusiasts • Housing assistance resources may be available through the facility for travelers who need support securing accommodations
