Job Title Registered Nurse – Case Manager Location Eureka, CA | Acute Care Schedule / Job Type Contract | Days, 5x8 Hours (8:00–16:30) | Every Other Weekend Required | Onsite Key Responsibilities • Provide case management services within an acute care hospital setting • Coordinate patient care plans and facilitate appropriate discharge planning • Collaborate with interdisciplinary teams to support patient transitions and continuity of care • Document all case management activities using Epic electronic health records • Float between like units based on facility need Required Qualifications • Active Registered Nurse (RN) license — compact license must be in hand at time of submission • Minimum of 1 year of RN experience required; 2 years preferred • Acute care experience required • Proficiency with Epic charting system • Must commit to working 3 of the 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) • Must live at least 50 miles from the facility to be eligible as a standard traveler; local candidates within 50 miles may be considered Preferred Experience / Skills • 2 or more years of case management experience in a hospital or acute care environment • Experience with discharge planning and interdisciplinary care coordination • Familiarity with floating between similar units in a hospital system Why Join Us • Located in Eureka, CA — within easy driving distance of 7 state and national parks, offering access to the rugged California coast, Redwood forests, and unique Victorian architecture • Free parking available, and housing assistance resources may be available through the facility for travelers who need support finding accommodations
