As a hospice CNA, you will provide the hands-on personal care that helps patients feel comfortable, clean, and cared for wherever they call home, whether that's a private residence, assisted living, or a care facility. You'll work closely with a dedicated RN Case Manager as a valued member of our interdisciplinary care team, in a supportive setting with clear expectations and reasonable caseloads. It's one less thing to worry about, so you can focus on your patients. \n Responsibilities: • Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care • Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation • Assist with dining and nutrition • Review and follow each patient's plan of care under the direction of the RN Case Manager • Provide comfort and companionship to patients and support to their families • Maintain electronic medical records Qualifications: • Active Certified Nursing Assistant (CNA) certification (required) • Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus • Current CPR certification (preferred) • Ability to work in an electronic medical record system • Strong communication skills, including written, verbal, and active listening • Excellent customer service, problem-solving, and time management skills • Compassion, patience, and the ability to handle emotional situations with care • Strong understanding of professional boundaries • Ability to lift 50 lbs. and good understanding of body mechanics • Valid driver's license and current auto insurance (required) Benefits: • Competitive pay • Fleet car program with fuel and insurance covered, for business and personal use *Based on eligibility • Company cell phone • Medical, Dental, and Vision insurance • Life insurance, Long term/Short term disability insurance • Generous PTO package • 401(k) with company match \n $18 - $23 an hour \n Join Our Team If you're ready to do meaningful work with a team that supports you, we'd love to hear from you.
