As a Hospice Social Worker, you'll support patients and families through one of life's hardest moments by helping them cope, plan, and find the resources they need. It's one less thing to worry about, so you can focus on your patients and families. \n Responsibilities: • Provide psychosocial support and counseling to patients and families throughout the hospice journey • Complete psychosocial assessments and document them in the patient's chart • Regularly assess and reassess the psychosocial, anticipatory grief, and pre-bereavement needs of patients and families • Carry out the Plan of Care, including social service goals, supportive counseling, problem solving, community referrals, and bereavement care • Connect families with community resources, including financial programs and eligibility support • Take part in weekly interdisciplinary team meetings • Help with funeral arrangements and provide counseling at the time of death when requested Qualifications: • Master's degree in Social Work preferred; Bachelor's degree in Social Work required • 1+ year of social work experience in a healthcare setting or social services agency • Current Social Worker license (required) • Hospice experience a plus • Knowledge of community resources, including state and federal financial programs and eligibility requirements • Understanding of hospice philosophy and the principles of death and dying • Strong organizational and communication skills • Valid driver's license and auto insurance (required) Benefits: • Competitive pay • Fleet car program with fuel and insurance covered, for business and personal use *Based on eligibility • Company phone • Medical, Dental, and Vision insurance • Life insurance, Long term/Short term disability insurance • Generous PTO package • 401(k) with company match \n $26 - $35 an hour \n Join Our Team If you're ready for a career that makes a real difference, we'd love to hear from you.
