POSITION SUMMARY: Provide support to patients to ensure compliance with care. Coordinate services and function as navigator and advocate in assisting patients to access services. Coordinate follow-up with patients for missed clinical appointments and work with them to overcome barriers to care. Ensure that appointments are scheduled and kept. Provide patient education appropriate to the needs of patients. EDUCATION AND EXPERIENCE: High School diploma or equivalent required. Healthcare education, certification, and/or training required. Outreach or customer service experience required. This is a part-time, 20 hours per week position. PHYSICAL DEMANDS: While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle objects; controls, reach with hands and arms; climb stairs; balance; stoop; kneel; talk or hear; taste or smell. The employee must occasionally lift up to 15 pounds.
