Working closely with physicians, nurses, and other healthcare professionals, you'll coordinate care, support safe discharges, and help patients navigate their healthcare journey from admission through discharge. Position Details: What You'll Do • Coordinate discharge planning and care transitions. • Collaborate with interdisciplinary teams to support patient needs. • Assist patients with complex medical and psychosocial needs. • Promote quality outcomes and efficient care coordination. • Uphold Mercy's mission, values, and service standards. Schedule • Preferred schedule: 8-hour shifts on Friday, Saturday, and Sunday. • May consider 12-hour shifts on Saturday and Sunday based on staffing needs. Qualifications Education • BSN required Licensure • Current RN license required. Experience • 2-3 years of acute care hospital experience preferred. • Care Management or Utilization Management experience preferred. Certifications • BLS required at hire or within 90 days. • Case Management certification preferred. Skills • Strong communication and critical thinking skills. • Effective time management and assessment abilities. • Ability to work collaboratively across all levels of the organization. • Knowledge of InterQual or Milliman Care Guidelines preferred. • Ability to provide care coordination across diverse patient populations.
