Job Title Registered Nurse – Home Health Location Saint Louis, MO | Home Health Schedule / Job Type Contract | Days, 5x8 (8:00–16:30) | Onsite/Field | Expected 40 hours/week Key Responsibilities • Conduct home health visits averaging 5–20 per day, meeting a daily productivity point target of 5.2 points, with visit types weighted accordingly (e.g., labs/lines, start-of-care OASIS assessments) • Provide skilled nursing care to a high-acuity, diverse patient population including phlebotomy, PleurX management, catheter care, port access and maintenance, PT/INR finger sticks, wound vac and wound care, ventilator management, and compression wraps • Support cardiac patients with infusion therapy (inotropes, dobutamine, milrinone), IVP Lasix, CADD pumps, handheld ECG monitoring, and telemetry interpretation • Deliver MS team services including antibiotic infusion, IV care, ostomy care, tube feeding, Foley care, disease management, and assessment of socioeconomic and medical needs with patient education • Complete OASIS assessments accurately and in compliance with home health documentation standards using EPIC • Participate in an every-4th-weekend rotation (both Saturday and Sunday) and an overnight on-call rotation of up to 5 nights per month; on-call nights are only scheduled on days the clinician is also scheduled to work Required Qualifications • Active RN license in Missouri or a qualifying compact state (submissions without MO license may be considered; compact license must match permanent residence state); Illinois license must be in hand at time of submittal if applicable • BLS certification required • Minimum 1 year of home health experience, OR minimum 1–2 years of med-surg or acute care experience • OASIS experience required • Home health experience required; candidates must be comfortable working independently with limited on-site resources • Must live more than 75 miles from the facility to be eligible; local candidates are not accepted Preferred Experience / Skills • Experience with cardiac infusion therapies including inotropes and IVP Lasix • Proficiency with EPIC electronic health records • Experience managing ventilator-dependent patients in a home or acute care setting • Familiarity with compression therapy, wound vac systems, and complex wound care protocols Why Join Us • Mileage reimbursement is available for field travel within the assigned territory (approximately 40–80 mile radius) • On-call scheduling is structured to be considerate of clinician workload — on-call nights are only assigned on days the clinician is scheduled to work, and weekend rotations include compensatory days off around the worked weekend
