As a Physical Therapist, you’ll contribute to our success in the following ways: • Evaluate patients and develop individualized treatment plans. • Provide physical therapy services in the home setting. • Monitor patient progress and adjust care plans as needed. • Collaborate with physicians, caregivers, and the interdisciplinary care team. • Complete timely and accurate clinical documentation. • Supervise therapy assistants and support staff, as appropriate.
