Employment Type: Full time Shift: Day Shift Description: Full-Time - 40 hours weekly; Days - 8 hour shift; New Hampton Medical Center This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for client’s home/residence. ESSENTIAL FUNCTIONS • Evaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client's home setting. • Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable. • Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician. • Communicates with physician any requests to revise the plan of care. • Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.
