Summary: The Hospice Bereavement Client- Coordinator coordinates the Hospice Bereavement activities, and provides psychosocial and bereavement counseling to the hospice patients and families and takes part in Bereavement Department programs Requirements • Licensed Master of Social Work, Licensed Clinical Social Worker or License Mental Health Counselor required. • Specialized training in the area of death and dying preferred or related field. • At least two years of hospice experience preferred. • Position requires ability to commute to multiple territories to ensure continuity of care. • A valid driver's license is required.(This requirement does not apply to positions assigned exclusively to Manhattan territory) • A vehicle is required to commute to patients.
