You will provide personalized support focused on comfort, dignity, and peace, partnering closely with patients, families, and our interdisciplinary care team to build and implement comprehensive, individualized care plans. Your clinical expertise and deep empathy will guide symptom management, ease transitions, and offer vital emotional support when it matters most .Key Responsibilities • Clinical Assessment & Monitoring: Assess patient healthcare needs, monitor symptoms, evaluate medication responses, and track overall progress. • Care Coordination: Collaborate seamlessly with physicians, family members, and interdisciplinary healthcare professionals. • Patient & Family Support: Provide compassionate education and emotional support to patients and their families throughout their journey. • Team Collaboration: Facilitate and actively participate in interdisciplinary team meetings to optimize care plans. • Documentation & Compliance: Ensure accurate, timely clinical documentation in alignment with company policies and regulatory standards.
