As part of our interdisciplinary team, you'll help individuals navigate complex emotional, social, and practical challenges while ensuring each patient's wishes, dignity, and quality of life remain at the center of care. What You'll Do As A Hospice Social Worker • Support patients and families through the emotional and practical challenges of serious illness. • Provide counseling, care planning, and resource coordination. • Help families navigate advance directives, care transitions, and end-of-life decisions. • Facilitate grief and bereavement support services. • Collaborate with an interdisciplinary hospice team to deliver compassionate, patient-centered care. • Document services and maintain compliance with hospice standards. • Participate in on-call coverage as scheduled.
