What You'll Do As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Requirements What You Bring • Active CNA License or Certification in the state of employment • Possess and maintains current CPR certification • One (1) + year Hospice, home health or related organization experience preferred • Comfort and sensitivity working with terminally ill patients and their families • A calm, dependable presence under emotionally demanding circumstances • Strong attention to detail in observation and documentation • A valid driver's license and willingness to travel locally for patient visits
