You'll have the opportunity to: • Build meaningful, long-term relationships with your patients. • Deliver evidence-based primary care supported by an integrated team of behavioral health providers, nutrition professionals, APPs, and care coordinators. • Practice proactive, preventive medicine that focuses on improving long-term health rather than simply treating illness. • Help shape and grow an innovative whole-person care model. • Work in a culture that values collaboration, compassion, accountability, continuous improvement, and clinical excellence. Position Summary The Primary Care Physician provides comprehensive outpatient primary care services, including preventive care, acute care, chronic disease management, and health promotion. The physician serves as the clinical leader for their patient panel while collaborating with Ascend's interdisciplinary team to develop individualized care plans that support whole-person health. Whole-Person Care at Ascend Whole-person care is more than a philosophy—it's how we practice medicine every day. We recognize that lasting health outcomes require addressing not only physical health, but also behavioral health, nutrition, lifestyle, prevention, and the social factors that influence well-being. Our physicians don't carry that responsibility alone. They work alongside an interdisciplinary team that expands what's possible for each patient, creating coordinated care plans that improve outcomes while strengthening the patient experience. This collaborative approach allows our physicians to practice medicine more comprehensively, support healthier patients, and build stronger relationships through truly integrated care. Who You Are You're a physician who believes healthcare is about more than treating disease.
