Your Impact (Core Responsibilities) • Medical Advocacy & Coordination: Track health outcomes, translate medical data into actionable interdisciplinary plans using our EHR, and occasionally attend local doctor appointments with clients to provide in-person advocacy. • Clinical Mentorship: Serve as the primary medical resource for non-medical staff, providing vital health-literacy training and weaving physical health strategies into behavioral care plans. • Consultative Care: Guide clients transitioning out of hospitals or emergency rooms to ensure safe, successful discharges and complete accurate medication reconciliations. • Safety Advisory: Provide clinical guidance during crisis situations and advise AWS management on health, safety, and infectious disease protocols.
