What You’ll Do As an LPN, you will work under the supervision of a Registered Nurse to provide quality nursing care and health support for individuals receiving services in residential and community-based settings. Responsibilities Include: • Administer medications and treatments • Monitor health conditions and report changes to clinical leadership • Assist individuals with activities of daily living (ADLs) • Maintain accurate clinical documentation and medication records • Support individualized healthcare and behavioral support plans • Promote a safe, respectful, and person-centered environment • Participate in interdisciplinary team collaboration • Respond to medical emergencies as needed • Reinforce infection control and basic healthcare practices with support staff. Candidates should be comfortable with frequent movement, travel within the assigned area, and direct client interaction. #INDNURSE Requirements What We’re Looking For Required Qualifications • Active LPN license • Graduate of an accredited LPN program • Current BLS certification • Valid driver’s license and reliable transportation Preferred Experience • Experience working with individuals with IDD/DD populations • Long-term care or community-based nursing experience • Experience with electronic health records (EHR)
