The LPN provides high-quality nursing care, administers medications and treatments, monitors resident conditions, and collaborates with the interdisciplinary team to support positive resident outcomes. Key Responsibilities • Administer medications and treatments as ordered. • Monitor residents and promptly communicate changes in condition. • Provide direct nursing care and support activities of daily living as needed. • Collaborate with physicians, nursing staff, and the interdisciplinary team on resident care plans. • Maintain accurate and timely clinical documentation. • Communicate effectively with residents, families, and healthcare providers. • Promote a safe, respectful, and resident-centered care environment. Requirements Qualifications & Requirements • Current, unencumbered West Virginia LPN license. • Skilled nursing or long-term care experience preferred. • Strong clinical judgment, communication, and organizational skills. • Ability to work independently and collaboratively as part of a care team. • Compassionate and resident-focused approach to care. • Ability to meet the physical requirements of the position, including lifting/moving 50 pounds or more and standing or walking for extended periods.
