Occupational Therapist - Monday to Friday / No Weekends KeystoneCare is seeking a dedicated Occupational Therapist for our Home Care division in the Philadelphia area. KeystoneCare is an Equal Opportunity Employer Responsibilities • Evaluate patients’ functional abilities and occupational therapy needs in the home setting. • Develop and implement individualized treatment plans focused on improving independence and quality of life. • Provide skilled OT interventions addressing ADLs, mobility, strength, coordination, endurance, and safety. • Assess the home environment and recommend appropriate adaptive equipment and safety modifications. • Educate patients, families, and caregivers on therapeutic exercises, techniques, equipment, and safety. • Monitor patient progress and modify treatment plans as clinically appropriate. • Complete accurate and timely clinical documentation. • Collaborate with the interdisciplinary team to coordinate patient care. • Communicate changes in patient condition and escalate clinical or safety concerns appropriately. • Participate in care planning and discharge planning as needed. • Maintain compliance with applicable regulatory, professional, and agency requirements. • Travel to patients’ homes throughout Philadelphia and surrounding counties. • Perform other duties as assigned. Requirements • Graduate of an accredited occupational therapy program • Current active state licensure as an Occupational Therapist in Pennsylvania • A minimum of one year of experience in a clinical or post-acute healthcare setting • Prior experience in home health care is highly desirable • Strong assessment, clinical, and organizational skills • Ability to create personalized treatment plans and adapt techniques as necessary • Valid Pennsylvania Driver's License and reliable transportation • Excellent interpersonal and communication skills • Ability to work collaboratively with multidisciplinary teams • Current professional liability insurance required • Ability to follow oral and written instructions effectively • Proof of Influenza Vaccination or an approved Medical or Religious exemption.
