Community Healthcare of Texas is currently recruiting for a Hospice Intake Coordinator. POSITION SUMMARY Provide patient admissions through coordination of referrals and admission team nurses. ESSENTIAL FUNCTIONS • Receive and verify validity of patient referrals. • Coordinate with admission team pertinent information in a timely manner. • Verify patient insurance. • Provide admission information to doctor's office, business office, admission nurse. • Various routine clerical duties and other assignments as needed.
