This is performed in accordance with Family Connects International model and OCHIDS documentation system. Essential Functions: • Provides services in accordance with the plan of care. • Makes the initial evaluation visit and completes documentation and follow up as dictated by the Family Connects model. • Refers patients to the appropriate resources as needed. • Provides those services requiring substantial specialized nursing skills. • Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy. • Coordinates services. • Informs patient’s PCP of changes in the condition and needs of the patient as appropriate. • Counsels the patient and family/significant others in meeting nursing and related needs. • Understands and adheres to established Agency policies and procedures. • Completes and submits OCHIDS documentation in accordance with Agency defined time frames. • Positively represents the department and Fisher-Titus in community interactions.
