Overview Rapidly growing! Urgently hiring! $1,500 sign-on bonus!!! The Skilled Nurse administers skilled nursing care to patients on an intermittent basis in the patients place of residence. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the RN. Services are furnished in accordance with Agency policies and procedures. We offer the following to our employees: • Be part of a unique healthcare company where we can help those in our communities who need our help the most • Flexible hours/work-life balance (full-time or part-time available) • Competitive pay • Paid on a weekly basis • Medical/dental/vision/life insurance • Paid holidays/401(k) match • Career growth opportunities • Great and collaborative work environment Responsibilities What You Will Do: • Administer medication in accordance with standards of practice. • Encourage social/rehabilitative skills and encourage self-care and independence. • Perform patient treatments in accordance with the plan of care. • Assist the patient in learning appropriate self-care techniques. • Prepare clinical and progress notes to document care provided • Monitor, document accordingly and report any change in patient condition to the Clinical Director. • Promote activities to achieve a safe and healthy environment. • Observe aseptic technique when performing patient procedures/care. • Practice Standard Precautions at all times. • Work with other members of the clinical team to develop and improve the plan of care. • Assist the patient in achieving established goals in the plan of care developed by the RN CM. • Maintain productivity according to the standards of the agency. • Participate in patient care meetings, in-services, orientation and staff meeting. • Understands when to request supervisory assistance. • Adhere to the policies and procedures of the agency. • Provide service in accordance with agency’s philosophy, standards, policies and scope of practice • Participates in Performance Improvement and adheres to HIPAA guidelines. • Other duties as assigned.
