Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible foreducation related to provided services. Qualifications Education and experience, essential • Must be a graduate of an approved school of clinical education • Must be licensed in the state where they currently practice. • Must have at least one year experience in home care. • Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice. • Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred • Demonstrated experience with and knowledge of hospice is preferred.
