SUMMARY Ensures that clients receive the services outlined in the Individual Habilitative Systems service plan in accordance with the policies of the agency, funder, licensing and accreditation entities. QUALIFICATIONS • Bachelor’s Degree in special education rehabilitation or a related human service field from an accredited college. • One (1) year QIDP experience required. • Current CPR certification. RESPONSIBILITIES • Work cooperatively with other department counselors and instructors, referral/sources, parents, guardians to maximize to client services. • Document the Individual Service Plan (ISP) for individuals receiving services made by medical and professional consultants. • Communicate with parents/guardians, peers, staff, and other community members regularly to maintain positive relationships and to ensure that service and support are implemented and continue to meet the individuals desired outcomes and needs. • Seek opportunities for community engagement and applicable external services based on client’s individual needs and in accordance to the individual service plan. • Provide staff in-service training.
