CareCollab is a Medicaid care management organization serving high-acuity populations across New York City, Westchester, and Nassau County. We work with individuals who have complex medical, behavioral health, and social needs, including serious mental illness, substance use disorders, chronic conditions, and housing instability. Through New York’s Health Home program, we provide care coordination services that help members: • Access medical and behavioral health care • Navigate social services and housing resources • Stay engaged in treatment • Avoid unnecessary hospitalizations and emergency room visits As a Housing Specialist, you’ll support members in securing and maintaining safe, stable, and affordable housing. This includes housing placement, rental assistance navigation, and eviction prevention for individuals experiencing housing instability. This role requires persistence, strong organization, and comfort working within complex housing systems while maintaining consistent in-office coordination. What You’ll Do • Support members through all stages of the housing process, from assessment through placement • Assist members facing rental arrears or risk of eviction by coordinating housing stabilization and eviction prevention resources • Prepare, submit, and track housing applications and required documentation • Coordinate housing interviews and prepare members for the interview process • Communicate with housing providers, landlords, and community partners as appropriate • Track housing goals and progress within members’ Plans of Care • Build and maintain relationships with housing providers and community-based organizations • Serve as a housing resource for CareCollab staff • Document all housing-related services accurately in designated systems and EHRs • Participate in care planning meetings and interdisciplinary case conferences What You Bring Experience • At least 1 year of experience in housing placement, housing navigation, eviction prevention, or supportive housing services including helping members navigate the legal housing system in NY, and accompanying them to court as needed • Experience working with rental assistance, HRA, housing stabilization, or eviction prevention programs preferred • Familiarity with NYC housing systems or subsidies strongly preferred Skills • Strong organizational and documentation skills • Ability to manage multiple applications, deadlines, and follow-ups • Comfortable working both in-office and in the field • Strong communication skills and ability to advocate for members • Proficiency with Microsoft Office and care management systems • Bilingual abilities are a plus This Role is a Good Fit If You: • Understand the realities and frustrations of housing systems including the eviction process, non-payment and holdover situations • Are comfortable working with high-need or housing-insecure populations • Can balance urgent housing crises with longer-term planning • Want mission-driven work with real responsibility Education • High School Diploma or equivalent required; Associate’s or Bachelor's degree preferred Why Join CareCollab • Mission-driven work addressing housing instability and eviction prevention • Supportive supervision and interdisciplinary collaboration • Hands-on impact with vulnerable populations • Opportunities to build deep expertise in housing navigation and stabilization
