The hospital is looking for Physician - Emergency Medicine in Eau Claire, WI Please review below details and let me know the best level of interest: JOB DESCRIPTION: SUBMISSION REQUIREMENTS: • ABEM/AOBEM Board Certificate or Board eligibility letter • Must have a clean file/background no malpractice • Procedure logs (last 2 years) - Updated CV, drivers license, color photo, state medical license (including licensure held in other states), CSR, DEA, education certificates, 5yr malpractice history, need narrative & legal documentation if provider has claims, ECFMG if applicable, ABEM/AOBEM Board Certificate, Board eligibility letter, TB/PPD/Quantiferon /Chest x-ray (within 12 months), COVID, and FLU (seasonal).
