SUBMISSION REQUIREMENTS • Must have active TX and DEA at time of presentation • Must have clean background and malpractice • Must be able to provide 4-6 shifts a month • Must cover all typical OB/GYN procedures • Must cover OBED and L&D AS A REMINDER: • All candidates must have meet the minimum shift availability listed on the job. • CV must have all dates in MM/YY to MM/YY format • Work History must be in Sequential order with most recent on top • Any Gaps over 30 days must be explained on the CV • EPA (Please use the attached EPA.
