Time 9am-5 pm, 8 Hours coverage Need to have NJ Med license, Fed DEA. Routine Outpatient Pediatrics. PPD: 18-22 SUBMISSION REQUIREMENTS • Must be Board Certified • Must have NJ license and DEA at time of presentation • Must have clean background and Malpractice history- if file has malpractice must email MSP coordinator for approval before presentation submitted. • Must be Board Certified • Must have NJ license at time of presentation*will accept non licensed candidates with IMLC* • Will accept providers that need to get a NJ Med license, Fed DEA. • Must have clean background and malpractice • Must submit EPA and NPDB with presentation • Must send availability with presentation AS A REMINDER: • All candidates must have met the minimum shift availability listed on the job. • CV must have all dates in MM/YY to MM/YY format • Work History must be in Sequential order with most recent on top • Any Gaps over 30 days must be explained on the CV • EPA (Please use the attached EPA.
