Assistance in Hospitalisation and Street Medical Care: Support clients in accessing emergency and routine medical care, including accompanying them to hospitals and coordinating with street medical teams. Promoting Hygiene and Sanitation: Ensure basic hygiene standards are maintained among the beneficiaries through education, provision of hygiene kits, and monitoring. Community Care Giver Development: Identify, develop, and coordinate a network of local community care givers to extend the reach of care and support. Follow-Up and Active Tracking: Monitor ongoing cases by maintaining regular contact, tracking progress, and ensuring continuity of care and services.
