Primary Duties and Responsibilities: ·Works collaboratively with all health care providers to optimize patient care. ·Advises physicians on the inherent dangers of drugs, incorrect prescription doses and their eventual side effects. • Promotes improvement in drug usage / dosage and in the supervision of therapeutic procedures for best results. • Coordinates the pharmacy duties, including the preparation, labeling and delivery of various medications or supplies according to prescriptions. • Ensures all dispensed drugs are recorded and delivered in accordance with local, state and federal laws or regulations. • Observes and treats patients in all health care settings and conducts research to further pharmaceutical care and advance the treatment and prevention of disease. • Individualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption. • Suggests improvement in work methods. • Conducts and evaluates medication histories, assesses compliance and suggests modifications to achieve desired outcomes. • Performs other duties as assigned. Knowledge, Skills, and Abilities: ·Strong work ethic and professionalism. • Ability to work independently. • Excellent oral and written communication skills. • Knowledgeable in all pharmacy procedures and regulations. Education Requirements (Essential Requirements): • PharmD Degree required. • Pharmacist license required. Work Experience Requirements (Essential Requirements): • Three years of relevant work experience required.
