We are currently seeking a Per Diem Registered Nurse for immediate hire to join our team in the Bronx and NYC. What You’ll Do: • Conduct Head-to-toe and Environmental assessments (Initial, Reassessment, PRN assessment, Supervisory visit) to identify patient-centered care needs. • Create a POC/ MO based on the assessment and the patient’s needs in collaboration with other patient healthcare teams. • Serve as an advocate for the patient to ensure continuity and quality of care. • Being ready to self-educate as needed to be proficient in documentation on different platforms to be compliant. • Be ready to educate patients/caregivers according to needs for safe patient care, including preventive care to avoid unnecessary hospitalization. • Work flexible hours, including evenings and weekends, as needed, to support our patients when they need you most.
