We are seeking an experienced, compassionate Nurse Practitioner or Physician Assistant to join our expanding team. This is an excellent opportunity for a provider seeking a high level of clinical autonomy, flexible scheduling, strong administrative support, and the chance to grow within an established and respected specialty group. About the Role Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout the Columbus area. You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while collaborating closely with facility nursing teams, primary care providers, and administration. What You'll Do • Perform on-site patient evaluations and follow-up visits • Manage diabetes, thyroid disorders, and other common endocrine conditions • Review blood glucose trends, laboratory results, and medication regimens • Optimize diabetes management and help reduce unnecessary finger sticks and medication burden • Collaborate with nursing staff, DONs, facility leadership, and primary care teams • Provide clear clinical recommendations and ongoing follow-up • Document in facility EMRs (training and administrative support provided) • Help reduce avoidable hospitalizations through proactive specialty care What We Offer • Competitive compensation: $150,000 – $220,000 annually • Flexible scheduling — no traditional office-based patient panel • Strong clinical and administrative support • Training and ongoing guidance in our clinical protocols and workflows • Significant clinical autonomy • Clinical leadership from Medical Director Dr. Don Zwickler, an endocrinologist with decades of experience • Opportunities for advancement as Elaya Health continues to expand • The chance to work with a respected, rapidly growing specialty practice Qualifications • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Ohio, or willingness to obtain licensure • Experience in endocrinology, internal medicine, geriatrics, or skilled nursing preferred • Endocrinology experience is a plus — not required for the right candidate • Strong clinical judgment and communication skills • Comfortable working independently while collaborating with a larger clinical team • Patient-centered and committed to high-quality care for the long-term care population How to Apply We would love to hear from you.
