The practice provides a full scope of internal medicine and geriatric primary care services in a variety of settings and locations. POSITION DESCRIPTION: This is a geriatric primary care role, taking care out to patients in assisted living facilities (ALFs) and other long-term care settings. The physician will be scheduled at one (1) building per day and will see 8 to 12 patients per day. The clinical duties of the role include, but are not limited to: - evaluation and management of geriatric patients - chronic disease management - annual check-ups - minor illness care The practice uses the Office Ally EHR.
