Position Overview We are seeking experienced and compassionate Registered Nurses (RNs) to perform in-home visits across all NYC boroughs. As a Fee-for-Service Registered Nurse, you will play a vital role in patient care, assessments, and care coordination—helping clients live safely and comfortably in their homes. Compensation (Per Diem Rates) • Initial Assessment Visit: $130 • Reassessment Visit: $100 • Resumption of Care Visit: $100 • Supervisory Visit: $80 Key Responsibilities • Conduct initial and follow-up in-home nursing assessments • Develop and update individualized care plans • Educate patients and families on disease management and self-care • Supervise and instruct caregivers and home health aides as needed • Complete timely and accurate documentation in compliance with regulations • Communicate effectively with interdisciplinary team members Qualifications • Active New York State Registered Nurse (RN) license • 1–2 years of experience in CHHA or long-term care preferred • Strong clinical assessment and documentation skills • Professional liability insurance is required • Reliable transportation and willingness to travel between boroughs Apply Today Join our dedicated team of Registered Nurses making a real difference in the lives of New Yorkers.
