We are seeking dedicated Registered Nurses (RNs) who are passionate about providing exceptional home health care to Adult Geriatric and Pediatric populations. Position Type: Per Diem (Fee for Service) Minimum Commitment: 25 patient visits per week Compensation: • $85 per routine visit • $95 per recertification/discharge • $120 per admission Responsibilities: • Provide skilled nursing care to patients in their homes across Adult Geriatric, Women's Health, and Pediatric populations. • Perform comprehensive patient assessments and develop individualized care plans. • Administer medications, treatments, and other physician-ordered interventions. • Complete admissions, recertifications, and discharge visits. • Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management. • Monitor patient progress and communicate changes in condition to the appropriate healthcare providers. • Maintain accurate, timely documentation in accordance with agency, state, and federal regulations. • Collaborate with physicians, therapists, social workers, and other interdisciplinary team members to ensure quality patient care. Qualifications: • Active Registered Nurse (RN) license in New York. • Experience in Adult Geriatric, Women's Health, and/or Pediatrics preferred. • Home health or community nursing experience is a plus. • Strong clinical judgment, assessment, and communication skills. • Ability to work independently and manage a flexible schedule. • Reliable transportation to travel throughout Brooklyn and Queens.
