This position offers the opportunity to provide comprehensive outpatient care for patients of all ages while working within a collaborative healthcare environment focused on preventive medicine, chronic disease management, and continuity of care. Responsibilities • Provide comprehensive primary care services for pediatric, adult, and geriatric patients. • Diagnose and treat acute and chronic medical conditions. • Perform preventive health screenings and annual wellness examinations. • Develop individualized treatment plans and coordinate specialty referrals when appropriate. • Educate patients regarding disease prevention, medications, nutrition, and healthy lifestyles. • Maintain accurate and timely medical documentation. • Participate in quality improvement initiatives and clinical team meetings.
