Our Foster Care Program is built on compassion, commitment, and care-and we're looking for a dedicated Case Planner to help us carry that mission forward. As a Case Planner, you will be a powerful advocate for children and families, guiding them through life's most challenging transitions and helping build brighter futures. You'll work closely with foster parents, biological families, and service providers to ensure each child receives the support they need to thrive. Responsibilities: • Manage a caseload of children in foster care, ensuring their safety and well-being • Develop and implement individualized treatment plans focused on permanency and stability • Support foster parents with parenting strategies and behavioral guidance • Maintain consistent communication with families, schools, and service providers • Represent cases in Family Court and ensure every child's voice is heard • Document progress and maintain compliance with agency and ACS standards Requirements: • BA / BSW and 2 years Human Services experience required. • Work experience in Child Welfare &/or Office of Mental Health System preferred • Computer literacy required • NYS driver's license preferred • Bilingual in Spanish preferred CMCS offers: • Medical, Dental & Vision Insurance • 403(b) Retirement Plan • Generous Paid Time Off • Flexible Work Schedules Make a Difference Every Day If you're ready to make a meaningful impact and be part of a compassionate, dedicated team, apply today and start your journey with CMCS! #HR24S
