The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team.  Schedule, Hours & Territory  • Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM  • Territory: Up to a 1 hour radius of Mishawaka, IN • RN Case Managers may participate in an on-call rotation with additional pay  • Some branches offer dedicated on-call staff, resulting in backup on-call coverage only  What We Offer  • Strong interdisciplinary team collaboration  • Supportive clinical leadership and resources  • Clear expectations and consistent workflows  • Environment focused on quality, compliance, and patient-centered care  Benefits  • Medical, dental, and vision coverage  • 401(k) with employer match at 100 percent on the first 3% and 50 percent on the next 2%  • Health Savings Account eligibility with employer contribution up to $500 for individual coverage and $1,000 for two-person or family coverage  • Company-paid Life and AD&D  • Voluntary disability, accident, critical illness, and hospital indemnity options  • Employee Assistance Program, WillPrep, and travel assistance resources  • Tuition reimbursement  • Paid time off and paid holidays  • Mileage reimbursement per company policy  Key Responsibilities  • Conduct comprehensive patient assessments and develop individualized plans of care  • Provide skilled nursing care in accordance with hospice standards and physician orders  • Coordinate care with physicians, caregivers, and interdisciplinary team members  • Educate patients and families on symptom management, disease progression, and end-of-life care  • Document assessments, interventions, and care plans accurately and timely in the EMR HCHB (Homecare Homebase)  • Participate in interdisciplinary team meetings and care planning  • Support after-hours patient needs as part of an on-call rotation when applicable  Qualifications: • Active Registered Nurse (RN) license  • Hospice or home health experience preferred  • Strong clinical assessment and care coordination skills  • Ability to work independently in the field  • Strong communication and documentation skills  • Experience using HCHB (Homecare Homebase) preferred  Field Role Requirements  • Active driver’s license  • Ability to pass a criminal background check  • Ability to pass a Motor Vehicle Record (MVR) check  • Current TB testing in accordance with agency policy  • Current CPR certification as required for the role
