Job Details: Job Location: 1946 Webster Avenue, Bronx, NY 10457 - Bronx, NY, Position Type: Per Diem, Education Level: Graduate Degree, Salary Range: $52.00 - $52.00Hourly, Position Title: Clinical Assessment Licensed Provider (OLP Clinician) Position Type: Per Diem, Hourly, Non-Exempt Schedule: About 5 hours/week Format: Hybrid Department/Program: Behavioral Health Services Location: 1946 Webster Ave., Bronx, NY 10457 Direct Supervisor: Director, Health Homes Care Management Direct Reports (if any): N/A   About Graham: Graham provides life-changing tools and resources for children, young adults and families who face some of the most difficult obstacles caused by poverty, racial injustice, and lack of access to educational opportunities, living wage employment, quality healthcare, and affordable housing.   We collaborate with communities and strong partners to create and implement innovative strategies so everyone can lead healthy, joyful, and successful lives.   Principal Objective/Role Overview: This NYS-licensed Clinician provides individual evaluation services to effectively engage and serve children, youth and families/caregivers to determine and/or maintain their eligibility to receive the range of supports provided by Graham’s Health Home Program.  The Clinician views each child/youth holistically and values the opportunity to meet the child/youth and their caregiver in their home, conducting a thorough, fully informed assessment of the strengths and needs of the client and their caregiver(s).     This individual is a New York State licensed practitioner of behavioral arts.    Job Responsibilities: Conducts home or community-based face-to-face clinical evaluations and assessments of children and youth, including collateral sessions with caregivers, as well as reviewing all available clinical notes and evaluations (in accordance with the Children and Family Treatment Support Services – Other Licensed Professional requirements and mandates).   Establishes Medical Necessity eligibility for kids and youth needing additional services.
