This role focuses on responding to urgent patient needs, providing symptom management, and offering emotional support during evenings, nights, weekends, and holidays. The On-Call Hospice RN works closely with the hospice care team to ensure continuity of care and timely response to patient and family needs. Qualifications Required • Active Registered Nurse (RN) license in the State of Connecticut • Current BLS certification • Minimum of 2 years hospice nursing experience Preferred • Home health experience • Strong clinical assessment and triage skills What You’ll Do • Respond to after-hours calls from hospice patients and families • Provide clinical triage and nursing guidance over the phone when appropriate • Conduct in-person visits for urgent patient needs when required • Assess patient symptoms and provide interventions to manage pain or discomfort • Provide emotional support and guidance to patients and caregivers • Communicate patient updates to the hospice care team and document all interactions • Coordinate care with physicians and other members of the hospice team • Ensure continuity of care and timely follow-up for patient needs Compensation Salary: $500 per week + fee per visit Apply Today!
