Community Care Coordinator We are seeking a compassionate and detail-oriented Community Care Coordinator to oversee daily resident care, assist with Activities of Daily Living (ADLs), and manage caregiver scheduling. Our Values: • Get Stuff Done • Residents Come First • Do the Right Thing • Lead with a Servant Heart • Unite as One Family • Guided by Faith Full-time employees enjoy a comprehensive benefits package, including: • Medical, Dental, & Vision Insurance • Life Insurance, AD&D, Disability Coverage • Accident, Critical Illness, Hospital Indemnity Insurance • Legal Benefits, Pet Insurance, Flexible Spending Accounts • 401k Key Responsibilities: • Assist residents with ADLs including bathing, dressing, grooming, toileting, and mobility. • Plan and post monthly schedules for CNA, Caregiver, Med-Tech, and Nursing staff. • Adjust staffing for call-offs and emergencies to ensure coverage. • Support recruitment efforts by scheduling interviews and assisting with onboarding. • Greet visitors, answer calls, and coordinate communication between residents, families, and staff. • Monitor resident health, respond to emergencies, and document changes. • Maintain accurate health records, medication logs, and supply inventories. • Participate in resident care conferences, activities, and welcome/orientation programs. • Ensure cleanliness and organization of resident rooms, dining areas, and common spaces. Qualifications: • High school diploma or equivalent. • CNA Certification (required). • CPR/First Aid Certification. • Strong communication, computer, and organizational skills. • Ability to work in a fast-paced, team-oriented environment. • 12 hours of annual continuing education (provided in-house).
