This role aims to enhance the quality of life for residents by providing personalized care plans and fostering a supportive environment. Key Responsibilities: - Conduct thorough health assessments and develop individualized care plans for residents. - Administer medications and treatments as prescribed, ensuring adherence to safety protocols. - Collaborate with interdisciplinary teams to coordinate care and optimize health outcomes for residents. - Educate residents and their families about health management, disease prevention, and wellness strategies. - Monitor and document residents' health status, reporting any changes to the appropriate healthcare professionals. - Participate in wellness programs and initiatives designed to improve resident health and well-being. - Provide compassionate and empathetic support to residents, addressing their health concerns and needs. - Maintain accurate and up-to-date medical records in compliance with regulatory standards. - Assist in training and mentoring junior nursing staff to uphold high standards of care. - Engage in continuous professional development to stay current with nursing practices and healthcare advancements. Qualifications: Required Education: - Bachelor of Science in Nursing (BSN) from an accredited institution. - Current Registered Nurse (RN) license in the state of practice. Required Experience: - Minimum of 2 years of clinical nursing experience, preferably in a wellness or geriatric setting. - Experience in developing and implementing wellness programs for seniors or similar populations. - Demonstrated experience in conducting health assessments and creating individualized care plans. Required Skills and Abilities: - Strong knowledge of geriatric health issues and wellness strategies. - Excellent interpersonal and communication skills to effectively interact with residents, families, and healthcare team members. - Ability to work independently and collaboratively within a multidisciplinary team. - Proficiency in electronic health records (EHR) and other healthcare management software. - Strong organizational skills with the ability to manage multiple priorities and tasks efficiently. - Compassionate and patient-centered approach to care with a focus on promoting health and well-being.
