Travel to provide care in patients' private homes and residential facilities throughout Greater Boston and parts of MetroWest. This position is eligible for a $5000 sign-on Bonus.   Job Responsibilities: The Weekend Nurse is an integral part of an interdisciplinary hospice team responsible for caring for patients at the end of life. Makes home and hospital visits, provides information sessions, evaluates new patients, facilitates admissions to hospice services, and coordinates care planning. Employs triage and assessment skills to titrate medications and manage symptoms appropriately and provides education to patients and families about terminal diseases and end-of-life processes. Communicates with an interdisciplinary hospice team about changes in patients' needs and outcomes of interventions.    Schedule and Weekly Hours: Saturdays, Sundays and Mondays, from 9 am to 9 pm (36 hours/week).   Education, Skills and Experience: Licensed Registered Nurse (RN) in Massachusetts or Licensed Practical Nurse (LPN).
