This is an on-site position located at Newton Wellesley Hospital.   Job Responsibilities: The position of Part-time Weekend Nurse Liaison is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital.    Schedule and Weekly Hours: 20-hour role.
