Generous Paid Time Off 401(k) Comprehensive Health Benefits Malpractice insurance provided License reimbursement and CEU support Paid training and onboarding Ongoing education and clinical support, including physician-level consult access Autonomy with Support to deliver independent care with access to physician-level consults.   Key Responsibilities Deliver person-centered care using evidence-based practices Collaborate with interdisciplinary facility teams Conduct assessments, develop treatment plans, and manage follow-up care Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs) Educate patients, families, and staff Complete timely documentation (within 24 hours) Compliance with facility, state and federal regulations/protocols. Qualifications: Qualifications: Active license as a Nurse Practitioner (NP)  Minimum 2 years of post-certification experience Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred Proficient with EHRs and clinical technology Strong communicator with a collaborative mindset
