In this role, you will provide compassionate hospice care to patients in their homes, visiting approximately five houses each day. This position offers a structured 9 AM to 5 PM schedule, Monday through Friday, allowing for a balanced work-life routine. Responsibilities • Provide high-quality hospice care to patients in their homes. • Visit approximately five patients daily, ensuring comfort and dignity. • Coordinate patient care and effectively communicate with the healthcare team. • Accurately document patient information and report changes in condition. • Travel between patient homes efficiently and safely. Requirements • Certified Nursing Assistant (CNA) with proven Home Hospice experience. • Ability to work full-time, Monday through Friday, 9 AM to 5 PM. • Valid driver’s license and reliable transportation for patient home visits. • Strong interpersonal and communication skills. • Compassionate and patient-centered approach to care.
