Physical Therapist, Home Health We're seeking a compassionate and skilled PT to join our home health team. This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals. Position Responsibilities: • Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs. • Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care. • Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities. • Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient. • Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan. • Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
