Siloam Hospitals Purwakarta, which has been fully accredited by KARS, provides comprehensive health services with excellence in Cardiology, Neurology and Trauma equipped with Catheterisation Laboratory (Cathlab), 64 Slices CT Scan, C-Arm, ESWL (Extracorporeal Shock Wave Lithotripsy), Endoscopy, Laparoscopy and other latest medical equipment Job Description • Provide comprehensive pharmaceutical care and services for inpatient patients in accordance with hospital standards, pharmaceutical practice standards, patient safety requirements, and applicable laws and regulations. • Review and validate medication orders and prescriptions, including medication appropriateness, dosage, route of administration, drug interactions, allergies, contraindications, and potential medication-related problems, and coordinate with physicians and other healthcare professionals when required. • Prepare, compound, dispense, and distribute medications and pharmaceutical supplies accurately and safely according to medication orders, including ensuring proper labeling, storage, handling, and timely delivery of medications to inpatient units. • Provide medication information and education to patients and families as needed, and collaborate with doctors, nurses, and other multidisciplinary teams in monitoring medication therapy, identifying adverse drug reactions, and optimizing patient treatment. • Maintain complete and accurate pharmaceutical documentation, medication records, and reports, while implementing medication safety, infection prevention, quality improvement, inventory control, and other procedures to support hospital-wide and department/unit targets.
